Cleveland, Ohio clinic performs US’s first face transplant

A team of eight transplant surgeons in Cleveland Clinic in Ohio, USA, led by reconstructive surgeon Dr. Maria Siemionow, age 58, have successfully performed the first almost total face transplant in the US, and the fourth globally, on a woman so horribly disfigured due to trauma, that cost her an eye. Two weeks ago Dr. Siemionow, in a 23-hour marathon surgery, replaced 80 percent of her face, by transplanting or grafting bone, nerve, blood vessels, muscles and skin harvested from a female donor’s cadaver.

The Clinic surgeons, in Wednesday’s news conference, described the details of the transplant but upon request, the team did not publish her name, age and cause of injury nor the donor’s identity. The patient’s family desired the reason for her transplant to remain confidential. The Los Angeles Times reported that the patient “had no upper jaw, nose, cheeks or lower eyelids and was unable to eat, talk, smile, smell or breathe on her own.” The clinic’s dermatology and plastic surgery chair, Francis Papay, described the nine hours phase of the procedure: “We transferred the skin, all the facial muscles in the upper face and mid-face, the upper lip, all of the nose, most of the sinuses around the nose, the upper jaw including the teeth, the facial nerve.” Thereafter, another team spent three hours sewing the woman’s blood vessels to that of the donor’s face to restore blood circulation, making the graft a success.

The New York Times reported that “three partial face transplants have been performed since 2005, two in France and one in China, all using facial tissue from a dead donor with permission from their families.” “Only the forehead, upper eyelids, lower lip, lower teeth and jaw are hers, the rest of her face comes from a cadaver; she could not eat on her own or breathe without a hole in her windpipe. About 77 square inches of tissue were transplanted from the donor,” it further described the details of the medical marvel. The patient, however, must take lifetime immunosuppressive drugs, also called antirejection drugs, which do not guarantee success. The transplant team said that in case of failure, it would replace the part with a skin graft taken from her own body.

Dr. Bohdan Pomahac, a Brigham and Women’s Hospital surgeon praised the recent medical development. “There are patients who can benefit tremendously from this. It’s great that it happened,” he said.

Leading bioethicist Arthur Caplan of the University of Pennsylvania withheld judgment on the Cleveland transplant amid grave concerns on the post-operation results. “The biggest ethical problem is dealing with failure — if your face rejects. It would be a living hell. If your face is falling off and you can’t eat and you can’t breathe and you’re suffering in a terrible manner that can’t be reversed, you need to put on the table assistance in dying. There are patients who can benefit tremendously from this. It’s great that it happened,” he said.

Dr Alex Clarke, of the Royal Free Hospital had praised the Clinic for its contribution to medicine. “It is a real step forward for people who have severe disfigurement and this operation has been done by a team who have really prepared and worked towards this for a number of years. These transplants have proven that the technical difficulties can be overcome and psychologically the patients are doing well. They have all have reacted positively and have begun to do things they were not able to before. All the things people thought were barriers to this kind of operations have been overcome,” she said.

The first partial face transplant surgery on a living human was performed on Isabelle Dinoire on November 27 2005, when she was 38, by Professor Bernard Devauchelle, assisted by Professor Jean-Michel Dubernard in Amiens, France. Her Labrador dog mauled her in May 2005. A triangle of face tissue including the nose and mouth was taken from a brain-dead female donor and grafted onto the patient. Scientists elsewhere have performed scalp and ear transplants. However, the claim is the first for a mouth and nose transplant. Experts say the mouth and nose are the most difficult parts of the face to transplant.

In 2004, the same Cleveland Clinic, became the first institution to approve this surgery and test it on cadavers. In October 2006, surgeon Peter Butler at London‘s Royal Free Hospital in the UK was given permission by the NHS ethics board to carry out a full face transplant. His team will select four adult patients (children cannot be selected due to concerns over consent), with operations being carried out at six month intervals. In March 2008, the treatment of 30-year-old neurofibromatosis victim Pascal Coler of France ended after having received what his doctors call the worlds first successful full face transplant.

Ethical concerns, psychological impact, problems relating to immunosuppression and consequences of technical failure have prevented teams from performing face transplant operations in the past, even though it has been technically possible to carry out such procedures for years.

Mr Iain Hutchison, of Barts and the London Hospital, warned of several problems with face transplants, such as blood vessels in the donated tissue clotting and immunosuppressants failing or increasing the patient’s risk of cancer. He also pointed out ethical issues with the fact that the procedure requires a “beating heart donor”. The transplant is carried out while the donor is brain dead, but still alive by use of a ventilator.

According to Stephen Wigmore, chair of British Transplantation Society’s ethics committee, it is unknown to what extent facial expressions will function in the long term. He said that it is not certain whether a patient could be left worse off in the case of a face transplant failing.

Mr Michael Earley, a member of the Royal College of Surgeon‘s facial transplantation working party, commented that if successful, the transplant would be “a major breakthrough in facial reconstruction” and “a major step forward for the facially disfigured.”

In Wednesday’s conference, Siemionow said “we know that there are so many patients there in their homes where they are hiding from society because they are afraid to walk to the grocery stores, they are afraid to go the the street.” “Our patient was called names and was humiliated. We very much hope that for this very special group of patients there is a hope that someday they will be able to go comfortably from their houses and enjoy the things we take for granted,” she added.

In response to the medical breakthrough, a British medical group led by Royal Free Hospital’s lead surgeon Dr Peter Butler, said they will finish the world’s first full face transplant within a year. “We hope to make an announcement about a full-face operation in the next 12 months. This latest operation shows how facial transplantation can help a particular group of the most severely facially injured people. These are people who would otherwise live a terrible twilight life, shut away from public gaze,” he said.

ABC to move Internet news network back to U.S. TVs

The U.S. television network ABC says its 24-hour news network, ABC News Now, will become a permanent 24-hour television news network starting in July. Currently, the operation is only available on the Internet as a streaming media web site.

The network started as a experiment by the Walt Disney Company-owned ABC in the summer of 2004 during the U.S. presidential campaign nomination conventions. It was mostly broadcast as a subchannel or shared channel within the bandwidth of an existing digital television (DTV) channel already owned by ABC. For instance if the local ABC affiliate is on Channel 7 with a DTV channel of 23, then ABC News Now may have been available on digital channel 23.1, sharing the same bandwidth.

Because few households in the U.S. are equipped to tune in digital over-the-air sub-channels, and virtually none are carried on local cable companies, ratings were low for the fledgling digital channel. Adding to that technical difficulty, cable carriage of the network was small. ABC News Now was delivered to only 6 of the 110 million U.S. households via cable. Most viewers of the network, 30 million at last count, watched the network via the Internet.

Citing money reasons, ABC shuttered the over-the-air ABC News Now broadcasts in January to focus on building the web-based service. But come July, ABC plans to return its news network and its 20 original programs to local TV stations. Without any commitment from national cable or satellite companies, ABC will return to digital sub-channels on local DTV transmissions.

“Anonymous” plans to protest Church of Scientology on February 10

The Internet-based group “Anonymous” released a new video on YouTube Monday, announcing international protests outside Church of Scientology centers set for February 10. The video “Call to Action” uses the same computer synthesized voice as a previous video titled “Message to Scientology”, which was posted to YouTube last Monday. The “Message to Scientology” video has been viewed over 1,400,000 times, and the “Call to Action” video has already been viewed over 237,000 times.

Anonymous is taking action against the Church of Scientology in response to what it sees as suppression of freedom of speech on the Internet. The group was first motivated after the Church of Scientology issued a copyright infringement claim to YouTube regarding a promotional video of Scientologist Tom Cruise speaking about his beliefs and using Scientology jargon. The Church of Scientology also issued a legal complaint to the website Gawker.com which is also hosting the video, but Gawker has stated it will not take the video down because it is newsworthy.

Anonymous set up a movement called “Project Chanology” to coordinate their efforts, and took down several Church of Scientology websites through denial-of-service attacks. Members of Anonymous have also participated in prank calls to Church of Scientology centers, as well as protests or “raids” outside Scientology buildings, and have posted some of their exploits to YouTube.

Anonymous is a collective of individuals united by an awareness that someone must do the right thing.

In the “Call to Action” video, Anonymous denies that they are composed solely of “super hackers”, stating “Anonymous is a collective of individuals united by an awareness that someone must do the right thing, that someone must bring light to the darkness, that someone must open the eyes of a public that has slumbered for far too long.” The video goes on to state “We want you to be aware of the very real dangers of Scientology,” citing what they term “gross human rights violations”.

Operation Freakout was the name of a Church of Scientology operation whose goal was to “get P.C. [Paulette Cooper] incarcerated in a mental institution or jail or at least to hit her so hard that she drops her attacks.” Paulette Cooper had written a book critical of Scientology called The Scandal of Scientology. Operation Snow White was the name of a Church of Scientology operation where members of the Church’s secretive Guardian’s Office infiltrated United States government agencies in Washington, D.C. including the I.R.S. The F.B.I.’s investigation into Operation Snow White led them to discover the planning of actions taken against Paulette Cooper.

The video concludes by inviting the viewer to “take up the banner of free speech” and protest with Anonymous world wide. In an email to CNET News, Anonymous stated that cities where unknown activities are planned on February 10 include New York City, Montreal, Houston, London, Melbourne, and Los Angeles.

On Friday, two spokespersons for the Church of Scientology commented on the recent actions of Anonymous. A spokesman told News.com.au “These types of people have got some wrong information about us,” and Church spokeswoman Yvette Shank told Sun Media that she regarded members of Anonymous as a “pathetic” group of “computer geeks”. On Monday, Radar reported that the Church of Scientology has asked the U.S. Attorney General‘s office in Los Angeles, the F.B.I., and the LAPD to start a criminal investigation of possible criminal activity related to the Internet attacks. A source told Radar that the Church of Scientology is arguing that the Internet attacks are a form of “illegal interference with business.” Radar also reports that the Church of Scientology is emphasizing its status as a religious organization in the United States, in order to assert that the Internet attacks can be classed as hate crimes.

In the video, Bunker states “I’m mainly concerned because you shouldn’t be doing things that are illegal, you just shouldn’t. It’s not morally right, and it’s not right when Scientology does it, it’s not right when we do it.” Bunker’s video post has been viewed over 156,000 times. On Sunday, Bunker posted a follow-up video to YouTube where he gives advice to Anonymous on how to peacefully picket. He also provides viewers with resources and weblinks to more information on how to hold a peaceful protest. He concludes this video by imitating Obi-Wan Kenobi‘s Jedi mind trick technique and voice from Star Wars, stating “And if I may, as Obi-Wan, tell you: You will do nothing illegal “.

Two Outer Hebrides councillors launch petition to retain local flight services

In the Outer Hebrides of Scotland, two councillors from the Scottish National Party (SNP) began a petition Friday, the day after Comhairle nan Eilean Siar — the local government council for the Western Isles — said it would discontinue flight services between the islands of Benbecula and Barra as part of its 2013 / 2014 budget.

The petition — entitled “To petition the Scottish Parliament that Comhairle nan Eilean Siar maintains its support for the lifeline air services between Stornoway, Benbecula and Barra” — has also been launched in response to another council decision to restrict flight services between the islands of Benbecula and Lewis to three days a week, although the move has been temporarily suspended for two weeks to allow for NHS Western Isles to provide funding for said services. Otherwise, the changes come into effect late next month. Gordon Murray and Rae MacKenzie, the two councillors who launched the petition, are both from the town of Stornoway in the isle of Lewis. At the council’s meeting, Councillor MacKenzie suggested an alternative budget retaining flight service financing. The petition says the council’s decisions “risk the future of all internal flights within the Outer Hebrides and would severely undermine the transport connections which have been built up between the islands over the last forty years, with serious implications for the local economy and community.”

The council’s decision to remove the £148,000 (about US$230,000 or €172,000) subsidy means Benbecula – Barra flights are to cease operation within the next few weeks. When the decision comes into effect, travelling between the islands will only be available through the use of ferry services. The council, which was trying to achieve about £5 million (US$7.75 million or €5.8 million) in spending reductions, said some 75% of seats on these flights were vacant.

Angus MacNeil, MP for Na h-Eileanan an Iar in the British Government, and Alasdair Allan, MSP for the same constituency in the Scottish Government, both of whom are SNP members, have criticised the council’s decision. MacNeil said he was “disappointed” about the move and claimed Comhairle nan Eilean Siar was “axing these services while pocketing the cash” provided to them by the Scottish Government. Meanwhile, Allan said the council’s action was “a significant blow to the community”. He said, from Benbecula: “As someone who used the Barra to Benbecula flight today, it is clear to me what importance this service has in keeping the Western Isles connected.”

Council leader Angus Campbell subsequently criticised MacNeil for his remarks, saying he was “extremely disappointed” by them. “The use of the term ‘pocketing the money’ shows a disrespect for the democratic process of local Government”, he said. He also requested that MacNeil and Allan “stand shoulder to shoulder with the Comhairle to campaign for increased funding for the Western Isles to help deal with these issues rather than turning on their democratically elected local council over individual issues they don’t particularly like.”

Campbell described the council’s 2013 / 2014 budget as “the most difficult we have faced during my time as a councillor.”

Australian government hopes to establish triage by phone

The Australian federal government hopes to slash hospital emergency department waiting queues by setting up a 24-hour national medical hotline.

A government source said that the National Health Call Centre Network would be manned by registered triage nurses 24-hours a day, 7-days a week. Triage nurses would perform a diagnosis over the phone based upon the description given by the patient. The patient would then be referred to the nearest emergency department, their local GP or pharmacy – as determined by the nurse.

The issue is expected to be discussed at next month’s Council of Australian Governments (COAG) meeting in Canberra. It is believed that the states and territories are supportive of the system.

If agreed upon by COAG, the service will be jointly funded by state/territory and the commonwealth governments at a cost of $40 million a year. The service would take 18 months to set up.

The service will be ran from a centralised call centre and be managed by a private contractor.

Julia Gillard, the opposition’s spokeswoman for health said any national call service needed to be linked with local GPs and medical services.

Gillard claims that under a Labor government, an after-hours “Pizza Hut” style service would be implemented, with a single national number connecting to a local call centre.

“You would be talking to people in the locality you are in and who know the local services,” she said

The Australian Medical Association, an organisation representing more than 27,000 doctors in Australia has slammed the proposal saying it will only deter people from seeking appropriate medical treatment.

Saturn moon Enceladus may have salty ocean

NASA’s Cassini–Huygens spacecraft has discovered evidence for a large-scale saltwater reservoir beneath the icy crust of Saturn’s moon Enceladus. The data came from the spacecraft’s direct analysis of salt-rich ice grains close to the jets ejected from the moon. The study has been published in this week’s edition of the journal Nature.

Data from Cassini’s cosmic dust analyzer show the grains expelled from fissures, known as tiger stripes, are relatively small and usually low in salt far away from the moon. Closer to the moon’s surface, Cassini found that relatively large grains rich with sodium and potassium dominate the plumes. The salt-rich particles have an “ocean-like” composition and indicate that most, if not all, of the expelled ice and water vapor comes from the evaporation of liquid salt-water. When water freezes, the salt is squeezed out, leaving pure water ice behind.

Cassini’s ultraviolet imaging spectrograph also recently obtained complementary results that support the presence of a subsurface ocean. A team of Cassini researchers led by Candice Hansen of the Planetary Science Institute in Tucson, Arizona, measured gas shooting out of distinct jets originating in the moon’s south polar region at five to eight times the speed of sound, several times faster than previously measured. These observations of distinct jets, from a 2010 flyby, are consistent with results showing a difference in composition of ice grains close to the moon’s surface and those that made it out to the E ring, the outermost ring that gets its material primarily from Enceladean jets. If the plumes emanated from ice, they should have very little salt in them.

“There currently is no plausible way to produce a steady outflow of salt-rich grains from solid ice across all the tiger stripes other than salt water under Enceladus’s icy surface,” said Frank Postberg, a Cassini team scientist at the University of Heidelberg in Germany.

The data suggests a layer of water between the moon’s rocky core and its icy mantle, possibly as deep as about 50 miles (80 kilometers) beneath the surface. As this water washes against the rocks, it dissolves salt compounds and rises through fractures in the overlying ice to form reserves nearer the surface. If the outermost layer cracks open, the decrease in pressure from these reserves to space causes a plume to shoot out. Roughly 400 pounds (200 kilograms) of water vapor is lost every second in the plumes, with smaller amounts being lost as ice grains. The team calculates the water reserves must have large evaporating surfaces, or they would freeze easily and stop the plumes.

“We imagine that between the ice and the ice core there is an ocean of depth and this is somehow connected to the surface reservoir,” added Postberg.

The Cassini mission discovered Enceladus’ water-vapor and ice jets in 2005. In 2009, scientists working with the cosmic dust analyzer examined some sodium salts found in ice grains of Saturn’s E ring but the link to subsurface salt water was not definitive. The new paper analyzes three Enceladus flybys in 2008 and 2009 with the same instrument, focusing on the composition of freshly ejected plume grains. In 2008, Cassini discovered a high “density of volatile gases, water vapor, carbon dioxide and carbon monoxide, as well as organic materials, some 20 times denser than expected” in geysers erupting from the moon. The icy particles hit the detector target at speeds between 15,000 and 39,000 MPH (23,000 and 63,000 KPH), vaporizing instantly. Electrical fields inside the cosmic dust analyzer separated the various constituents of the impact cloud.

“Enceladus has got warmth, water and organic chemicals, some of the essential building blocks needed for life,” said Dennis Matson in 2008, Cassini project scientist at NASA’s Jet Propulsion Laboratory in Pasadena, California.

“This finding is a crucial new piece of evidence showing that environmental conditions favorable to the emergence of life can be sustained on icy bodies orbiting gas giant planets,” said Nicolas Altobelli, the European Space Agency’s project scientist for Cassini.

“If there is water in such an unexpected place, it leaves possibility for the rest of the universe,” said Postberg.

Wikinews attends 2018 Bangalore ComicCon

On November 17 and November 18, Wikinews attended the 2018 Bangalore Comicbook Convention, in Karnataka, India. Beginning on Saturday morning, the ComicCon continued till Sunday. Organised by ComicCon India, who organised Comic Con in Hyderabad in October, this event was held at Karnataka Trade Promotion Organisation (KTPO)’s convention hall. Wikinews spoke to convention’s international guests Ryan O’Sullivan, Dan Watters, and Vanesa Del Ray.

UK-based freelancer comicbook writer Ryan O’Sullivan, who has worked for Image Comics and Vault Comics, and wrote video game comics including Eisenhorn, Warhammer 40,000, and Dark Souls, was one of the guests attending ComicCon. Along with him, London-based freelance comic book writer Dan Watters was also one of the attendants. Watters has worked with Image Comics on Limbo, as well as Lucifer for Vertigo Comics. Watters has also written comics for Assassin’s Creed, Wolfenstein and Deep Roots. Cuban comic book artist Vanesa Del Rey was also one of the guest attendants, who has illustrated Scarlet Witch, Daredevil Annual, and Spider-Women Alpha for Marvel Comics; and Redlands for Image Comics.

The convention started at 11 AM. Crossing between the genres of science fiction, horror, anime, fantasy and comics, the event sprawled throughout all corners of the facility. Marvel Zone, Amazon Video zone, AXN Live RED Special Session, Warner Bros’ Aquaman and Fantastic Beasts: Crimes of Grindelwald experience, and Sony Pix zone attracted large number of audience.

On day two, Sumit Kumar, creator of Chin Chin, inspired by Chacha Chaudhary and Tintin, had a panel in the afternoon.

At 1:30 PM, there was a video presentation by Yali Dream Creations. The video was about The Village. Yali Dream Creations’ Shamik Das Gupta said the story of their comic The Village is “supernatural, social, horror”. Later, there was another presentation about their other comic, Rakshak ((hi))Hindi language: ?Protector — A Hero Among Us, about a masked vigilante in India. The first part of the four-part comic was released in 2016’s Delhi ComicCon, Gupta said. Gupta also added “Delhi gang rape story pushed me”. The session ended around 1:45 PM.

After Yali Dream Creations, there was a comic launch by Rahil Mohin. Mohin had previously launched Sufi Comics in 2008, The Wise Fool of Baghdad and two comics about Persian poet Rumi and now Blame it on Rahil. While he was on the panel, he was sketching cartoon, while the host was asking questions that Mohin had prepared, focused on the 90s cartoon themes. The questions were “[What was the] breed of Courage the Cowardly Dog?”, “Catchphrase of Flintstone”, and “Arch nemesis of Dexter”. His session finished by 2 PM.

At 2 PM, there was a session with AXN, and questions about AXN shows. Musician Shrey also played scores of various AXN shows on guitar, which audience had to guess. At 2:30 PM, there was a session with Cuban comic artist Vanesa Del Rey. She said it was her first convention in India, and said, “It is like any major convention in the US”. This year, Vanesa Del Rey was nominated for an Eisner Award. During the discussion, she said she wanted to do sci-fi epic,romance, mystery and thriller comics in future. Regarding romance genre in comics, she said it is “something worth exploring”. And regarding her career choice, she said her grandmother was an artist and her “family was very supportive”.

Later, there was a session with Sony PIX, with Minions appearing on the stage.

The event finished around 8 PM.
